We are pleased that you have chosen The Dentists on Pearl as your new dental providers. We promise to help you achieve a healthy, functional, and comfortable smile.
At your first dental visit we will need some basic information to help establish you as a new patient. Please complete our New Patient Form and Medical History Form and bring with you to your first appointment to save time. Upon arrival you will be asked to complete a medical history form if you haven’t done so already. Please bring a list of your current medications.
Please also bring your current insurance information.
Since your appointment time has been reserved just for you, please notify us at least 24 hours in advance if you are unable to make your appointment.
At your first visit you will be given a thorough exam to evaluate your dental health. This exam includes but is not limited to:
Oral cancer screening
Occlusion (bite) evaluation
TMJ evaluation
Screening for cavities
Evaluating for periodontal disease
A full set of radiographs will be taken to help evaluate and diagnosis any current and potential issues that a clinical exam may not reveal.
If you have recently had dental x-rays taken at another office please ask them to forward a digital copy to our office at office@TheDentistsonPearlVT.com.
